Oh, I don't know, he looks perfectly sane to me... 
The eyes in the background give for a creepy effect, but I note that a lot of the proportions are off for him. While I get the exaggerated smile for affect, the eye is wider than the corresponding eyepatch, and too large for the face you've given him and makes the nose look offcentered even thought it's where it should be. Similarly, the shoulders are too narrow.
